7. If they had young children, were their visitation / access issues / restrictions?

8. Did they experience any recent family loss (loved one, friend, pet etc.)?

3. If yes, do you know what type of drugs they used?

4. If yes, do you know how often they used drugs?

6. Did substance use ever affect their work or family relations?

7. If yes, were there warnings or interventions by their employer (if employed)?

8. Did you notice them either drink more, or consume more drugs (or both), in the lead up to their death?
